The Select Form
This form is used to begin and accomplish many of your tasks. You enter criteria
here that specifies the selection of employers.
Creating a query using the Select Form is quite easy. A number of fields are
listed on the form. If all fields are left empty, the results will include every
record (not recommended). If you enter a value into a field, the results will
include all records that match that value. For example, if you enter
"POLK" into the county field and "01" into the effective
month field, the results will include employers located in Polk county which
have a policy effective date in January.
Buttons, Buttons, Buttons
In the select form you will notice a number of buttons to click. The following
is an explanation:
The
Build Button Located adjacent to a number of fields on the
select form; clicking this will activate a pop-up form that allows you to
select and deselect values available to the particular field the button is
located adjacent to.
The Close Form Button Located on all forms; this button will close
the active form on your screen.
The
Save Button Located on the form; this button will save the
current selection as it appears on the select form. This allows you to run a
particular query in the future without having to recreate it. When activated, a
pop-up form will allow you to name the selection for your future reference.
The
Record Selectors Located at the bottom of the form as arrows;
these six buttons allow you to navigate through your previously saved
selections, create new records, and delete unwanted records.
The
Carriers Build Button Located next to the Carrier Field; this
button will pop-up a form listing all of the workers compensation insurance
carriers providing coverage to your state. This form will allow you to lookup
the carrier codes to enter into the carrier field on the select form.
The
SIC Codes Build Button Located next to the SIC Code field; this
button will pop up a form listing all of the SIC Codes and their corresponding
descriptions.
The
Class Code Build Button Located next to the Class Code field;
this button will popup a form listing all of the workers comp class codes and
the descriptions. Texas, Pennsylvania-Delaware, and New Jersey Only.
The
Clear Button Located on the form; this button will initialize
the current selection on the select form in order to start a new query from
scratch.
